Some of the qualifying conditions for medical cannabis in the various states are:

What to send us to get an appointment for your medical marijuana permit:
Before we can schedule your appointment, we need documentation of your qualifying medical condition. What
we need is chart notes (also called SOAP notes) from several recent visits with your medical doctor (MD) or
a doctor of osteopathy (DO), and any recent imaging results. Fill out the Authorization to Disclose Records
form (next page). Bring, fax, or mail it to your doctor's office, or just give it to the person at the front desk. You do not need to talk to your doctor at all, just the records clerk. Your doctor's office will then fax the necessary records to us. It is rare that we need more than 20 pages or so. Records must be less than three years old. We cannot accept notes from a nurse, physician's assistant, naturopath, chiropractor, optometrist, podiatrist, etc. After we receive your records, which can take several weeks, we will review them to be sure that you are qualified. This can take up to a week. We will then call you to set up an appointment to see one of our doctors. Your appointment with us could last as long as four hours. (Most last about two hours.) Please schedule your day accordingly. The best tips for making your appointment run smoothly are:
Show up on time.
Pay in advance by phone when you make your appointment.
Fees to see the doctor vary by state. In most states, the fee is $200, with a discount to $150 if you are low
income. Oregon fee is $180, low income $120. If you pay in advance, you get a small discount. We accept
major credit/debit cards, cash, or money orders. We do not accept personal checks. Your insurance will not
cover this. There is also a state registration fee in most states.
We accept the following as documentation of low income:
Food stamp award letter with current date. (The food stamp card by itself is not current documentation)
Supplemental Security Income statement with current date.
Tax return or Social Security income report showing income under $1200 per month, or $2000 per month for a
household of two or more persons. You must have documentation with you to receive the low income scale.
The Hemp and Cannabis Foundation is not a dispensary. No medicine or plants are distributed at our offices,
ever. After you receive your legal permit, we will do our best to put you in touch with patient organizations in
your area. THCF is not affiliated with them. Once you have your permit, you are free to contact them. THCF
