Where to start? It's not getting enough light, all the growth is stretching trying to get closer to the light. I'm guessing ph is out of range, there could be root damage .

I agree on the light, something is awfully wrong...

Poor thing does look sad. May be worth investing in a cheap led light they can be had for around $75. Get way less stretch at least.
