Michigan Medical Marijuana, Medical marijuana clinics, Medical Marijuana Certification, Marijuana Certification Center, Michigan marijuana clinics, Michigan medical doctors, medical marijuana benefits, Medical Marijuana Prescription print out the medical release form fill it out send it in of fax to clinic they will get your medical records from ur doctor and if you qualify they will set u up with apptment it cost 200 but you can pay installments on the apptment and at the apptment you will get the doctors certification then after that you have to fill out appt for state id card and send that to the state.

If you go to the michigan(dot)gov/mdch web page it has the forms there - but most of the Dr.s that write the certifications will have a copy of the forms available. I know a Dr in the Northern Lower Peninsula that holds certification clinics. Have you asked you current Dr about it? Unless you are a VA patient then any Michigan licensed Dr (MD or DO) is able to write the certification request (but most at this point are not willing). If you go to a MMMJ specialist Dr then you will need the last 2 years of medical records, and usually $200 for the visit. When you send in your papers, you will need $100 to send in with your paperwork ($25 if you are on SSI, SSD or Medicaid).
